The public is invited to join members of the Healthy Fond du Lac County 2020 Steering Committee as they provide an update on its successes and next steps during an informative breakfast on Thursday, April 30. The breakfast is scheduled from 7 to 8 a.m. in the Agnesian HealthCare Plaza Level Conference Center, 430 E. Division Street in Fond du Lac. Key leaders from the community will discuss important health priorities and share their unique insights on population health. New data from the 2014 community health assessment will also be shared. The event will feature the following presenters: Rick Parks, Society Insurance; Michael DeGere, DPM, FASPS, FACFAS, vice president of Population Health Management, Agnesian HealthCare; and Heather Schmidt, DO, Agnesian Health & Wellness medical director. Business and community leaders throughout Fond du Lac County have come together – as part of the Healthy Fond du Lac County 2020 initiative – recognizing that today’s health needs have become more complex and expansive than ever before. While good health cannot be forced upon the public, educated and enlightened citizens can and do solve their own health concerns and those of their families and communities. The Healthy Fond du Lac County 2020 Steering Committee has been executing the 2012-2015 Fond du Lac County Health Improvement Plan, which establishes goals and measurable objectives to address the four most pressing health concerns in the county. They include the increasing rates of obesity; access to dental care for individuals with Medicaid; access to mental health services; and alcohol use and abuse facing Fond du Lac County.
