Join us in strategizing our 2022-2024 Community Health Improvement Plan for Fond du Lac County! 3 different dates and topics, attend one or all three.

• Oct. 4th, 7:00-8:30am – Mental Health (Fond du Lac)

• Oct. 5th, 7:00-8:30am – Chronic Disease (Fond du Lac)

o   Location: Marian University Stayer Center, S National Ave, Fond du Lac, WI 54935

• Oct. 6th, 8:00-9:30am – Substance Use (Ripon)

o   Location: Ripon College Pickard Commons, 304 Elm St, Ripon, WI 54971

Event Description:
Fond du Lac County Health Department and SSM Health, in collaboration with Healthy Fond du Lac County partners, are hosting 3 community strategizing sessions to discuss community goals, strategies and ways we can make a collective impact to improve the quality of life and address top health needs in our community. We encourage local businesses, healthcare, non-profits, educators, government, community/service groups, residents, and more to attend these sessions and learn how they can be active partners in strategies identified. These sessions are free and open to the public.
