Lakeland Care, Inc., a local organization that provides long-term care services and support to elders and adults with disabilities through Wisconsin’s Family Care program, is excited to announce the expansion of its Family Care services to nine additional counties – Adams, Waushara, Marquette, Green Lake, Columbia, Dodge, Dane, Jefferson, and Rock – starting January 1, 2025. The expansion supports Lakeland Care’s continued growth in creating a world we all want to live in by empowering individuals, strengthening communities, and inspiring futures.  Family Care is a Medicaid program designed to assist eligible elders and adults with disabilities live independently at home and within their community. To qualify for Family Care, members must meet financial and functional criteria as assessed by the local Aging and Disability Resource Center (ADRC).  The expansion into the nine Wisconsin counties represents a significant milestone for Lakeland Care.  Since its founding in 2000, Lakeland Care has been dedicated to meeting the evolving needs of local communities through innovative support and tailored solutions.
