Starting Wednesday Fond du lac County residents are being asked to participate in a community health survey. Public Health Officer Kim Mueller says the health department, Agnesian Healthcare, Fond du lac School District, United Way, YMCA and other groups have partnered to conduct the phone survey. Mueller says information will supplement a community health assessment done six years ago to allow officials to analyze trends over time. Mueller says the survey will take about 15-20 minutes and will be conducted by an independent research firm that specializes in community surveys.
